HI ,

if the patient seen in my hospital first visit in inpatient after 5days the same patient is coming to outpatient of same Doctor for follow-up , what we should code for second visit in outpatient - new or establish, kindly me advice please.

Established. Even though it's the patient's first visit with this specific doctor in the outpatient setting, this visit will be established. The place of service is irrelevant.
