Patient was admitted to the Observation on 1/31/2016. The admitting physician gave the orders over the phone on that day at 5:50pm. He saw the patient the next morning (2/1/2016) and did the H&P. Then, patient was discharged from the Observation on the same day at 4:10pm.
How should it be reported to Medicare?
 
I would do 99218,99219,99220 for observation different day admit and 99217 observation discharge. Even though the admission was within a 24 hour period it was still two different days (at least that's the way it was explained to me). The code for same day admission/discharge is 99234, 99235, 99236.
