Wiki INR G0250 denials

We are getting denials on all of our G0250 claims:

Benefit maximum for this time period or occurrence has been reached.

We have only billed G0250, one unit, once every 30 days (after 4 tests have been performed)

Anyone have any idea what we are doing wrong??
 
INR billing

Christy Neff, Did you figure out the reason for the denials? We are going to start billing for the INR and my understanding is we can bill G0250, one unit, for every 4 test every 28 days. Which seems like you were following the guidelines.
