What date of service do you use when submitting the initial code of 99363? Totally stumped.
 
99363 is intended to be used at the conclusion of the initial 90 days of outpatient therapy.
If the management ends with a final time period of less than 60 outpatient days, then it cannot be reported with 99363 or 99364.
We bill it on the last dos in the 90 day period.
