Ins Appeal Letters

I’m new to this specialty (DPM or Ortho surg) and would appreciate your help with appeal letters. If you have any templates or useful resources—like websites or tips—you could share, I’d be so grateful!

I would be happy to help with specific appeal guidance. Still, I constantly modify our appeal letters based on the patient's policy vs. provider/hospital contracts, state vs. federal laws, CMS policy manuals, LCD, and NCDs. A "General" or "Universal" Appeal letter is not best practice. Common appeals trending lately are downcoding, DRG los transfers, CCI editing, Medical Ness, or no auth when there is auth, noncovered when it's covered, and timely filing.

Just google this. It's faster and easier. Most specialty societies have examples for common ones.
If you are coming into a new role or new group, they don't already have templates?

I agree with @tpereto , you can't really use a one size fits all or cookie cutter template.

It also goes without saying, before appealing, make sure you understand the denial and that it's really something that should be appealed. My first question was always, was this correctly coded and billed in the first place. In my experience managing A/R teams, many times folks would waste a lot of time trying to appeal something that should not have been billed in the first place, or may have been incorrectly coded or billed. Not saying that we don't have a large amount of claims to appeal, just that make sure you are spending that time correctly.
