I'm trying to find the correct coding for insertion of a left ventricular vent only (not an assist device) placed on a date after Ecmo cannulation (36822) on a pediatric patient on ECMO support in an attempt to "optimize recovery of his LV". Here are the details:

"...Sternum was opened. Pledget pursestring placed and vent placed with extreme care taken to avoid entry of air into the heart. Likewise extreme care was taken to establish the Y connection to the circuit with perfect deairing. Copious irrigation and suctioning were completed. Drains were reinserted. Snares were again tucked under the sternum away from the heart and great vessels. I would also add that I carefully examined the aortic root. The L button was visualized and appeared nicely distended....I elected to leave the sternum open, as child was not bleeding, to ensure no negative impact on the heart..."
