Insurance denying E0562 due to incorrect modifier

BCBS began denying HCPCs E0562 when billed w/ E0601 due to incorrect modifier. All other insurances accept E0562 w/ NU KX, but not BCBS. EOB states "denied b/c we have not received the requested modifier code to process the claim." Billing as outpatient and professional. Please advise, I have 5 claims stuck due to the same situation.
