dr here(hospital) is doing an angioplasty of both the arterial and venous aspect of an a/v graft(fistula), two seperate punctures. am i allowed to code for both? 35475 and 35476.

When coding for both venous and arterial, you can only code for the venous.
so with 2 puncture sites your codes would be:
36147
36148
35476
75978
so with 2 puncture sites your codes would be:
