Does anyone know if there is a list of base time for the Regular Preventative Codes like the Medicare codes have? I cannot find one anywhere to see when the G0513 would come into effect with the Preventative.

I don't know of any such list.

I would love to get your source (a link or the title of the document and who published it would be great) for this timetable because I would love to have the formal documentation of this information.

Do you mean for 99381-99387 and 99391-99397?

Time isn't a factor for selecting those codes, so I would be surprised if a table like you posted above existed for those codes.
