What exactly is coded for inpatient? Obviously I know ICD-9, but what procedure codes are used? Is an e/m code billed for each day the visit is there?

Are you talking about inpatient physician charges (such as hospital rounds or surgeries) or inpatient facility charges (what's billed from the hospital on the UB)?
