We received a request to authorize a neurostimulator procedure on the peripheral nerves, CPT code 64555 X2 and L8680, implantable neurostimulator electrode x16. This is for a Medicare advantage patient and everything I am reading isn't very clear to me if this is a covered service. The provider wants it denied if it's not covered. Can anyone help with this? I did see where the 64555 shouldn't be billed more than once. I attached the request, minus patient info of course. I would appreciate any help!
