I have a case where the patient has a hemi arthroplasty placed because of a fx. The code that was originally used was 27236. They are now coming back in because of a dislocation on this hemi hip. My provider did a closed reduction under sedation not general anesthesia. My question is, am I allowed to report this closed reduction with 27265 or is this something else?

there are two parts to this case. it appears that after this closed reduction, it turned out that the reduction was not going to work because this dislocated again. so my provider ended up doing a revision on the hemi to a total hip.
I am thinking that I can report this with 27134-58 (or possible a 78, but leaning more towards a 58.) I have another person saying that this would be reported with 27132-conversion. I am not totally sure about this since the original procedure was a hemi hip.
I am unsure on the correct way to look at this, so if anyone has any advice will be greatly appreciated!! PLEASE help!!
