MACRA deregulated the sustainable growth rate (SGR) formula used since the Clinton era to calculate physician fee-for-service payment updates. That's the part said to be a "doc fix," as physicians no longer face the snowballing pay cut the SGR was creating. MACRA also instituted a payment system based on performance: The Quality Payment Program's two tracks -- MIPS or Advanced APMs -- rolled separate value-based reporting programs (EHR, PQRS, VM) into one. Did this consolidation "fix" anything? The process of reporting may not be any simpler for providers, but CMS has made the requirements to earn positive payment adjustments very easy. As quality reporting through MIPS or an Advanced APM is the only way eligible clinicians will earn Medicare payment updates beginning in 2019, it's essential to get on board.
