Let's say that a clinician states in the Assessment: Type 2 diabetes with neuropathy, with long term insulin, type 2 diabetes with hyperglycemia, with long term insulin use, acquired absense of right leg, below knee along with other diagnoses. The plan is listed below all of the diagnoses and states "Plan, See patient instructions for diabetes related to insulin orders when fasting." The HPI does not talk about the diabetes, neuropathy or hyperglycemia. Is what you see below enough to code the diabetes with complications?

Assessment:
Type 2 diabetes with neuropathy, with long term insulin
Type 2 diabetes with hyperglycemia, with long term insulin use
Acquired absense of right leg, below knee
Hypertension

Plan: See patient instructions for diabetes related to insulin orders when fasting.
