If the dx is wrist pain - the pt comes in 5 days postop with no complaints. Denies numbness or pain. Does this count as the status of a chronic condition. Following these notes is the physical exam.

This is a Medicare pt. outside of the global period but having subsequent visits, therefore I am having to use an E/M code. I'm thinking that in this scenario it's a PF hx...then I will establish exam and decision-making. I don't do much E/M coding. I appreciate your assistance.
