I ran into this the other day, Ischemic cardiomyopathy is NOT an HCC, however, Ischemic Congestive Cardiomyopathy is. When reading through the ECHO and it states that they have ischemic cardiomyopathy with heart failure with PEF, would we code this as ischemic congestive cardiomyopathy or is it not the same? Looking for some clarification. Thank you!

I would code ischemic cardiomyopathy (I25.5) and heart failure with preserved ejection fraction (I50.30) as the echo states. The HCC for heart failure trumps the HCC for congestive cardiomyopathy anyway so no loss there. I don't see how you could make the jump to ischemic congestive cardiomyopathy from the stated echo findings.
