What would you suggest to code when your doctor documented both ischemic heart disease 414.9 and dilated cardiomyopathy 425.4?

I hate to say this word but would you assume the patient would then have ischemic cardiomyopathy 414.8 or would you code the two codes above separate? Thanks for the help.
 
can't assume since the physician didn't link those conditions together. it may be best to query the physician and ask if the conditions are linked since there is one code to represent them together. if not then I would code them separately
