issues with 92014 and 92250 " bundling"

I do CT for a Optometry office. All of a sudden, I started seeing Anthem and Aetna denying 92250 and 92014 due to bundling.
 
When I started doing optometry coding last year, I was instructed that any additional optometry services (like 92250) would bump the visit out of the comprehensive eye visits (92014) and into an E/M visit.
so from what I am hering is to use 99213 with modifier 25 ( if turned medically) and 92250 XS ( aetna always preferred RT LT) there has been new NCCI edits with Aetn/anthem with bundling that I recently found out. But there is no clear answer how to bill it turned medically. I just do not want to do the wrong thing and cause a audit
