Our physician has started using I-Stent along with his cataract surgeries at our hospital. We are billing procedure code 0191T for the Insertion and C1783 for the device. But we are getting denied for medical necessity for the I-Stent . We have the appropriate dx codes for the 0191T procedure and it gets paid with no problem. Is the C1783 billable separately? Should we be getting an ABN and collect this cost from the patient? Any guidance/help is greatly appreciated.
 
The 0191T covers both the insertion of the iStent and the device itself. The fee bundles the two, kind of like how the punctual plug materials fee is bundled with the insertion code.
