A patient comes in for an IUD insertion, status post cytotec the night before due to stenotic cervix. IUD insertion was attempted, but US confirmed that the placement was not right (it was in lower uterine segment/cervical os) so the IUD was removed in the same visit.

I could really use some help with billing this one. Do I still charge for the IUD itself, even though it was removed prior to the patient leaving?

Also, would I bill the insertion with a discontinued procedure modifier, or would I bill insertion and removal?

HELP! And Thank you in advance for any advice/assistance.
 
You would bill for the insertion and removal since the IUD was inserted. Usually the company will send a replacement IUD when something like this happens; just give them a call and explain. I would use dx 996.32 along with the V25.13 to indicate a problem with the IUD. bill for the US as well.
