IV Hydration was started in our CAH ED upon patient's arrival. Hydration ran for 47 minutes while patient was in the ED. Patient was sent out by life flight with running IV solution being sent with patient. Nurse was unsure to document stop time, due to patient continuing onto higher level of care with IV solution running. Question: Can the nurse document a stop time for this IV solution as the time patient was transferred/discharged?
 
The nurse should not document a stop time if the IV was not stopped. The documentation should rather show that the IV was still running at the time of discharge, and then the facility can use the discharge time as if it were a stop time in order to code for the full amount of time that the IV was running while the patient was in the facility. But the documentation should be accurate as to what was actually happening.
