We want to start doing IV infusion in our office and I want to make sure I am coding this correctly since this is new to me. We put the medication in the IV bag and then insert into the patient. I have been coding:

96374
96375 x 4
j0610- Calcium
j3415- Pyridoxine
j3420- B12
j3475- Magnesium
j5050- Normal Saline

If medications are mixed into a bag, then they are coded as infusion (96365 -initial hour, 96366 additional hours) if a start and stop time were documented. If they were pushed into the IV line, they are as you have coded above, except Normal Saline is only ever coded as hydration (96360 and 96361). Were all the above really combined in one IV bag and hung with a drip rate?
 
Yes. They want to combine all medication in the IV bag and hung with a drip rate. If we use code 96365 do we still charge for the medication we put into the IV bag?

Yes, you absolutely do. If it runs for 16 minutes or longer code 96365 for first hour. If it runs for an additional hour, then you code a 96366. Again, start and stop times are imperative Good luck, coding infusion can be tough!
