Procedure

Indication:
Peripheral IV Start Site: right antecubital

Catheter Size: 20-gauge
Catheter Type: angiocath
Venous Access:.
IV insertion was successful after 1 attempt(s). Positive blood return was noted. The site was taped securely after insertion.
Person Attempting:
Manufacturer/Lot #/Exp. Date: 5% Dextrose and 0.9% Sodium Chloride, Manufacturer ICU Medical, Lot# 5809075, Exp Apr 2024.
fluids were infused at a rate of 200 ml/hr ml/hr. Infusion started at 1400. Infusion completed at 1415.
Rocephin was administered, at a rate of mixed with 50 cc d5 0.9NACL at 200 ml/hr, started at 1400., stopped at 1415.
Patient Status:. the patient tolerated the procedure well.
Peripheral Post-Infusion Status: There was no complications, but no extravasation, no redness and no edema.
