Pt is given 4 different drugs. Two of them by IV infusion and two of them by IV push. Would the correct coding for each drug be 96365 for the initial infusion and 96367 for the subsequent infusion and 96374 for the initial IV push and 96375 for the subsequent IV push. I am confused as to how this would be coded for each drug since 96365 and 96374 are both initial but they are both different methods of giving the drugs. Thanks for any help. I am also billing for all 4 of the drugs.
 
Initial Service: Initial service code is reported only 1 time per encounter and the subsequent, sequential and concurrent codes are reported regardless of the subsection in which the initial service code appears.

When administering multiple infusions, injections or combinations, only 1 "initial" service code should be reported, unless protocol requires that 2 separate IV sites must be utilized
To report 2 different "initial service codes use Modifier -59

The initial code should be selected using the following hierarchy:
1st Chemotherapy
2nd Therapeutic, prophylactic and diagnostic services
3rd Hydration

Administration route:
1st infusion
2nd pushes
3rd injections

Infusions

You cannot have a 96365 and a 96374 on the same visit. Code 96365 as the initial medication given 16 min or longer, the rapid push as 96375 if each is a a different medicaiton or 96375 and 96376 if teh same medications. The same medication given several times must be at least 30min after the initial of the medication of like name.
i.e.: Demoral - IV push at 0800, and 1300
Amoxicillin - 16 minutes at 0900
96365 - amoxicillin
96375 - demoral 0800
96376 - demoral 1300

orAmoxicillin - 16 min
Demoral IV push at 800
Zofran IV push at 1000
96365 - amoxicillin
96375 X 2 for the demoral and zofran
