The patient received the following drug administrations:

Rocephin 1/23/2020 @ 1607 - site: Left Hand --> 96374
Rocephin 1/24/2020 @ 1006 - site Right Forearm --> ???
Zithromax 1/24/2020 @ 1037 - site: Right Forearm --> 96374-59 (XS)

Would the second Rocephin IVP be coded as 96376 or 96375-59 (XS)?

They were given on different dates of service so each would have their own code. And are you coding the charges for the medications too? What site was used seems irrelevant to me unless the same IV was used since you could only code one IV access for both drugs.
I haven't looked in my book but be sure you're coding the correct admin code depending on route of admin.

96374

Rocephin 1/23/2020----96374
Rocephin 1/24/2020----96374 (first given on this DOS)
Zithromax 1/24/2020---96375

Double check that these are actually IVP rather than infusions (left hand definitely sounds like infusion.). If they are infusions, but do not have stop times, then you can still bill them with the IVP codes.

My apologies, I forgot to mention that the patient was in OBS status at the time of these administrations, which is why the different sites matter.
