Hello,

I have a doctor that wants to bill for a female patient for IVF with ICSI S4015, 89280 with the diagnosis N46.8 (male infertility). Now this is a male diag. Am I correct to say that this will be rejected by the insurance company?

What diag. would be suggested for this case. The male had a vasectomy reversal so they are giving him the infertility not the female.

Any help would be appreciated to get this billed out correctly without a denial.
