If pt. received Velcade 2.9mg for 30mins would i code as follows:

IVP 96409 and J9041x29 units?

I'm confused about the J9041? the billing unit is 0.1mg however, vial size is 3.5mg?

Confused on how to code the J code.

GM,

If the vial size is 3.5, then the billing units would be J9041 x 35 units. Was any of this drug wasted, due to patient not being able to tolerate the drug or having side effects?

FYI - when the vial size is 3.5 mg I've only seen billable units of 35.

It would depend on how much of the vial was used. You can' t code the entire vial if part of it would be used for another patient. You need to check with your pharmacist or nursing manager to find this out.
