I'm having a hard time looking this one up. The code the providers have been using is J1000, but that's for "Injection, depo-estradiol cypionate, up to 5 mg". Is there a different J code for this?
You could do code J1050 and use 40 as your units. Also I've found the best way is to look at the actual vial/syringe and it will list an NDC #, than look it up by cross referencing the NDC.
Thanks for your input. The NDC never appeared on the electronic claim. I'm trying to determine the correct NDC, and also make sure we used the correct code and # of units.
