Wiki J codes for Avastin changes?

Does anyone have any insight on J code changes for billing Avastin for ocular use in an office?
J3490 was always used per FCSO Medicare guidelines for Florida. J9035 was used for chemotherapy reporting.
Now I am hearing of J7999 is the current recognized code for Avastin administered in an office setting
for ocular use. I did see this listed on an LCD article on FCSO portal.
Can anyone confirm or deny this change and when is affective?
