J1335 Ertapenem sodium Aetna Medicare Denial

Does anyone have any idea why J1335 (ertapenem sodium) would be getting denied by Aetna Medicare as CO96? I cannot identify a trend as far as DX goes. We do have a few that are paying but most are denying CO96. We are only billing 2 units per day with the NDC removed and itemized by individual DOS. This is for home infusion as well POS 12.
