Has anyone else received denials from Medicare since 10/01/20 on CPT J1756 Venofer, we are billing with CKD and iron deficiency anemia. It seems like since 10/01/20 we have been receiving denials on CKD patients with N18.3 or lower. I have not been able to find any update with Medicare. Also I am currently in Jurisdiction F with Medicare. Has anyone found an article on this update or now why I am receiving so many denials?
