Hi all,
We have several denials for J2405 HCPC for Zofran which started around the around August. When looked up, the brand name Zofran has been discontinued. Should the description now be the generic of ondansetron in order to be paid? The diagnosis has not been changed on our side so we are puzzled.

Check your NDC #. If you have NDC #s hard coded to your HCPCS make sure that it has been updated.
