what would the correct unit count be if provider wrote "inject 0.5 cc Kenalog (50 mg /5 mL) mixed with 0.5 cc lidocaine 2% (20 mg/1 mL) " I bill code J3304 - ML but what is the unit count please
 
J3304 is for injection of Zilretta. If you're injecting Kenalog, check to be sure you're not supposed to be reporting J3301. (HCPCS is clear about what should be reported based on trade names).
In the case of cc and mL....it's a 1:1 ratio. So 0.5cc = 0.5 ML. That means that with 50 mg of Kenalog in 5 mL you'd have 5 mg of Kenalog within a half cc/mL of volume. (you don't report or count the lidocaine). If Zilretta is your code, you'd bill 5 units. since that's reported by single mg of weight of medication.
