We performed Pump Refill below and bill to MDCR
  • J3490 ITDS- Unlisted.
  • 95991 ITDS- Pump refill.
  • 76942 ECHO GUIDE FOR BIOPSY
* J3490 denied unprocessable with REASON short description: Hydromorphone 200 mg

Pump Refill:
Details Patient was lying on the table in a supine position. The pump area in the LLQ was prepped with betadine solution x3. The area was draped sterilely. The pump template was placed. A 22G Huber needle was inserted through the access port until the back wall was contacted. A total of 4ml of Dilaudid was removed. 20ml of Dilaudid 10mg/ml was infused into the pump reservoir. The Huber needle was removed. The pump was reprogrammed to 0.49 mg/day of Dilaudid. He tolerated the procedure well without any complaints. He was briefly observed and subsequently discharged.
