A given drug can have multiple NDC numbers depending on the packaging and supplier. You should use the NDC number from the packaging of the product that was given to the patient at this encounter - it would be on the box or the vial from which the drug was drawn and should be included in the patient's record. Most practices have the providers scan or paste this information into the chart at the time of the administration.We had a patient who came in to receive IM injection of Dupixent 400 mg. Codes that were submitted were J3490 (Unclassified drugs) and 96372 for the administration fee. Claim being denied for NDC # missing but I'm having trouble finding the NDC # for this code. Has anyone submitted for this and know the NDC #?
