J9267 paclitaxel infusion with mod JZ as no waste drug was made is denied by BCBS. They responded that "the procedure code is inconsistent with the modifier used or required modifier is missing".
if somebody know new rules or changes in this J code, I'll appreciate your help. Thank you.
 
What is the NDC code that was billed with J9267? According to the NDC directory Paclitaxel comes in single & multiuse vials. If the NDC code submitted on the claim was for a multiuse vial then it would be inappropriate to bill JZ as it is only applicable to single use vials of drugs.
