Looking for billing guidelines for 2014 and 2015 for codes 88305,88312,88313,88341,88342,88343. Also can any shed light on the daily maximums that can be billed to insurance.

most insurance companies follow the Medicare MUE policy on the number of units you can bill...UHC has thier own policy on this however. you can find both of them on the internet...if you have access to dennis padgett, he has a MUE table in his pathology coding handbook.
