Good Afternoon - does anyone know when to append a JW modifier for billing waste for chemo?

The JW modifier is not always required when billing for waste, it is a carrier preference. If you are going to bill using the JW modifier, you would report the amount of drug that was given to the patient (J-Code and correct units) on one line of the claim and then on another line, you would report the J-Code with a JW modifier and then the correct units that identify the amount of drug that was wasted.
 
The only drugs that you may bill with a JW modifier are single dose vials. Multi-dose vial waste may never be billed to any patient. Our carrier for medicare also requires that the waste be billed on a seperate line item with the JW appended to that line.
