Wiki JZ/JW Modifier HELP

I am in GA. We are having the hardest time discerning which payers are following the Medicare Part B JZ/JW modifier requirements. Also for the list Medicare has supplied, they state this is NOT an all-inclusive listing. What is everyone else doing with these? We are at wits end.

You're absolutely right—navigating the JW and JZ modifier requirements under Medicare Part B can be incredibly frustrating, especially when CMS explicitly states that their list of applicable HCPCS codes is not all-inclusive.

1. JW Modifier: Used when some of a single-dose drug is discarded. You must report the amount administered and the amount discarded separately.
2. JZ Modifier: Required when no drug is discarded from a single-dose container. This became mandatory for all applicable claims starting

   • CMS provides a non-exhaustive list of HCPCS codes that may require JW or JZ modifiers, depending on the setting.
   • Claims without either modifier for single-dose drugs are often rejected or returned as unprocessable .
   • The requirement applies to all separately payable drugs under Medicare Part B with status indicators G, K, or K2

The reason the list CANNOT be all-inclusive is because HCPCS codes are updated quarterly so there are constantly going to be new & deleted codes, as well as updates to existing codes.

The list we have for JZ/JW is Medicare, Humana, UHC, Aetna, and now Wellmark. Cigna is questionable, as they want to know wasted drug with JW, but I don't see that they want JZ. Let me know if you know otherwise.
