"A skin incision was made in line with the previous scar from the inferior pole of his patella down to the tibial tubercle. This was taken down through subcutaneous tissue and thickened peritenon, some minor scar tissue in this area and palpable sutures just proximal to the tibial tubercle and some mild thickening at the previous rupture site, but it appeared to have healed well. Careful debridement did reveal the suture knots. These were debrided out and removed and then the areas of thickened tendon consistent with tendinitis were treated by a tenotomy method. A 22-gauge needle was used to carefully fenestrate the tendon up and down the length, particularly in the areas of the thickened tendon consistent with tendinitis. This was to slightly loosen up and debride the area with fenestration or tenotomy type technique and induce a healing reaction in this area."

27680: Tenolysis, flexor or extensor tendon, leg and/or ankle; single, each tendon

Would it be appropriate to use this CPT code as the patellar tendon attaches to the tibia and is an extensor tendon?

Thoughts please.

Thank you.
