Therapy services for all outpatient services including hospital based, will fall under therapy cap limitations. Once the cap is reached, we will need to use the kx modifier. I am trying to hard code the chargemaster - so does the GO, GP, GN - go before the KX? :confused:
 
KX Modifier

Per CMS the order doesn't matter. KX has been longstanding in use for the cap on the outpatient side. Hospital outpatient is just being added to the mix.
