Wiki L4 to Ilium instrumentation

My physician only placed wires and screws from L4 to ilium. The instrumentation codes I normally use are "add on" codes. Can anyone tell me what I should use here?
 
I believe there are previous threads with similar questions. From what I remember reading, if the primary code that an add-on code is typically used with, isn't billed for the same DOS, you'll use the unlisted code from that section.
