EXAM:
Ultrasound and fluoroscopic guided sclerotherapy of peritoneal/labial
varices
HISTORY:
Enlarged and painful varices
MEDICATION:
2.5 mg Versed
75 mg fentanyl
1 g Ancef

Time of sedation:
1 hour

Intravenous conscious sedation was administered by a dedicated
independent observer with continuous hemodynamic and respiratory
monitoring performed, including the use of pulse oximetry.
FLUOROSCOPY TIME:
2.6 minutes
DAP:
0.9 gray per centimeter squared

TECHNIQUE/FINDINGS:

Informed consent was obtained from the patient detailing the risks,
benefits and alternatives to pelvic vein embolization.

The patient was transported to the fluoroscopy suite and placed
supine. A timeout procedure was conducted. The labial region was
prepped and draped in standard sterile fashion. All elements of
maximal sterile barrier technique were followed including cap and
mask, sterile gown, sterile gloves, large sterile sheet, hand hygiene
and 2% chlorhexidine for cutaneous antisepsis.

Using ultrasound guidance the enlarged labial veins were accessed with
a 25-gauge needle. Gentle contrast injection confirmed intraluminal
position and volume of needed sclerosant.

Two injections were performed on the right using a mixture of
contrast, normal saline, air and 1% Sotradecol. A single injection was
made on the left.

A compression bandage was placed.

Patient tolerated the procedure well and there were no immediate
complications.


IMPRESSION:
Successful ultrasound and fluoroscopic guided sclerotherapy of labial
varices.
