Hello!
I work for an independent lab that receives biopsies from physicians all over the US. We do not receive biopsies from other laboratories. Lately we’ve been inundated with rejections from Anthem/BCBS to bill our local even though we’re required to bill the state in which the biopsy was taken per the 2012 BCBS mandate for ancillary claims.
We’re currently billing our information in Box 32. I’ll also mention that we’re billing POS 11, 19 and 22.
Anyone w/ the same issue or know why we are being flooded w/ rejections?
