If a single laceration (10.5 cm) covering both the scalp and the forehead is repaired, should it be coded 12004 or 12015? or split by site?

Can you get the note?

If you can get the full note and know how it's divided, I'd code it based on where the majority of the wound was located.

Basically I'm looking to see if it's a scalp wound that extends onto the forehead. Or a forehead wound that extends into the scalp.

Hope that makes sense.
