Procedure:
3. Laminectomy of L5 with partial laminectomy of L4 and partial
laminectomy of S1, for complete decompression of the spinal canal and the
nerve roots.
4. Complete diskectomy, L4-5 and L5-S1.
Laminectomy was then performed at L5 with partial laminectomy of L4 and
partial laminectomy of S1 to completely decompress the spinal canal and
the nerve roots and allow partial medial facetectomies and foraminotomies.
This was all performed by combination of use of the bone scalpel to cut
the lamina, followed by use of rongeurs to remove the cut lamina, followed
by Kerrison rongeurs, and curettes to remove the additional bony material.
Kerrison rongeurs were used for the partial medial facetectomies and
foraminotomies to decompress the nerve roots taking great care to protect
the nerve roots from any unnecessary stress or trauma. Once this was
completed, the nerve roots were gently retracted medially at L4-5 and
L5-S1 individually and this allowed access to the disk space. Diskectomy
was then performed bilaterally using pituitary rongeurs, Kerrison
rongeurs, and curettes to completely remove all the disk material.
I am leaning towards 63030-50 (L4-5)
63035-50 (L5-S1)
