Did the Dr. do the procedure for stenosis or herniated lumbar disc? Did he specify what nerve roots were decompressed?
 
Code 63047 is for unilateral or bilateral and if he did procedure for stenosis I would bill the 63047. When deciding to bill per segment or per interspace, I go by nerve roots decompressed. Because he doesnt really specify which nerve roots decompressed, I would bill per interspace, 63047 for L3-L4 and 63048 for L4-L5. If he had stated in the OR that he decompressed the L3, L4, L5 nerve roots I would bill 63047 and 63048 x 2.
