we have a patient who needs to have an old lap band removed, the lap band has no port. She will have a new lap band with all the components placed during the same session. i have no idea was code to use for the removal of the lap band with no port. can someone help me ?

you could use removal of foreign body but my guess insurance won't pay because surgeon has to remove old in order to insert new and it would be considered bundled which brings us to CPT 43659 (per CPT12 guidelines).It includes removal and replacement. I would use it because you are removing an old gastric band and replacing it with a new one that has port.
