Can anyone help point me in the right direction of where I can obtain the LCD for biopsies? I cannot find this. We are having a difficult time with denials with biopsies when reported with R21 (Rash). Our providers obviously have no idea what the rash is. This is why they are submitting the biopsy. But, most carriers state this is not an acceptable diagnosis code.
