Wiki LCD issues for PT-Help pls!

I am having trouble with only 1 of my pt claim rejecting. CPT code 97033 is being used and ICD 9 727.06, 719.7, 845.00, 719.47 are being used. The rejection is saying LCD issues, "covered for specific conditions which are not meet" . Pt has medicare ins. Can anyone help me with what this means? Are there specific codes that need to be used for this CPT. I have searched Medicare website and i can not pin point a direct answer. Thanks :mad::confused:
